How to find your code
Grab the Explanation of Benefits your insurance sent you. People call it an EOB. Look for a column named Adjustment Reason, Remark Code, or Claim Status. The code might be a number like 45. It might mix letters and numbers, like N130. It might be letters only.
Type the code just like it looks on your EOB. Numbers, letters, or both. Leave the dash in or take it out. Prefixes like PR, CO, and OA are handled for you.
